This is the 9th in a series of videos - 'deconstructing cataract surgery' - designed to help surgeons in training develop a solid foundation in cataract surgery technique. Supporting with the index finger opposite your entry site, 'the digital technique', has advantages of being very comfortable for the patient under topical anaesthetic, atraumatic, and ensuring that the pressure in the anterior chamber is maintained during keratome entry. Here we argue for a two-step, limbal, temporal main entry site placement. Avoid posterior pressure during keratome entry to avoid aqueous escape, and support with the thumb of your left (non-dominant) hand for added control.
