Per OSHA 1910.119 PSM standards an initial Process Hazard Analysis (PHA) must be conducted on processes covered by the standard. At least every five years after the completion of the initial PHA, the PHA shall be updated and revalidated. The standard allows for a number of methodologies to be implemented in order to meet the requirement. Notable potential methodologies include Hazard and Operability Studies (HAZOPs), What-Ifs, and Checklists.
