Identifies self by name. Identifies resident by name. Washes hands. Explains procedure to resident, speaking clearly, slowly, and directly. Maintains face-to-face contact whenever possible. Provides privacy. Adjusts the bed to a safe level. Locks bed wheels. Places blanket over resident and removes top bedding and gown while keeping resident covered. Fills basin and checks temperature (105°F - 115°F). Has resident test water temperature and adjusts if necessary. Asks and helps resident to participate in washing. Uncovers only one part of the body at a time. Places a towel under the body part being washed. Washes, rinses and dries one part of the body at a time. Washes areas appropriate to the partial bath; face, axilla, hands, and peri care. Eyes, Face, Ears, and Neck: Washes eyes with wet washcloth (no soap) using a different area of the washcloth for each eye, washing inner area to outer area. Asks resident if they would like soap on their face. Washes the face from the middle outward, using firm but gentle strokes. Washes ears, behind the ears, and the neck. Rinses and pats dry. Washes resident’s axilla. Rinses and pats dry, repeat on the other side. Washes resident’s hand. Rinses and pats dry, repeat on the other side. Changes water. Washes hands, applies gloves, and provides front and back peri care. Removes and discards gloves. Washes hands. Puts clean gown on resident and assists with grooming as necessary. Removes blanket and return bedding. Returns bed to lowest position. Places call light within resident’s reach. Disposes of wash water, cleans equipment and returns to proper place. Closes and ties top of linen bag, touching only outside. Disposes of linen per facility policy. Washes hands. Documents procedure.
