The clinical presentation typically includes altered mental status, hypothermia, bradycardia, hypotension, and distinctive skin changes, creating diagnostic challenges that may overlap with other conditions. Early recognition is crucial, particularly in elderly patients presenting with these symptoms, as delayed intervention significantly increases mortality risk. Management requires a multidimensional approach beginning with airway protection and hemodynamic stabilization. Airway management must account for potential laryngeal edema complicating intubation. Intravenous fluid resuscitation addresses hypotension but requires caution in patients with cardiac comorbidities. A critical component often overlooked is the concurrent administration of hydrocortisone to address potential adrenal insufficiency before thyroid hormone replacement. Thyroid hormone replacement with intravenous levothyroxine (200-400 mcg) forms the cornerstone of therapy, with T3 supplementation remaining controversial. Identifying and treating precipitating factors such as infections, cardiac events, or medication interactions is essential for recovery. Supportive care includes passive rewarming, electrolyte correction, and intensive monitoring, often necessitating ICU admission. Case studies illustrate variable presentations and outcomes, emphasizing the importance of maintaining a high index of suspicion across diverse patient populations. When recognized and treated promptly, patients can experience remarkable recovery, highlighting the need for rapid, comprehensive management protocols and ongoing medical education to address this life-threatening condition.
