Derek wants to marry Meredith. His mom has given him "The Ring" and told him that Meredith is "the one". Derek carries it about in his pocket, planning to propose. Then a pregnant patient of Derek's dies, due to his surgical mistake. Derek doesn't know what he wants from anyone, anymore.
