For the past two years, the US Coast Guard has been examining every aspect of Oceangate’s operations, to determine what caused the Titan tragedy and who, if anyone, should be held accountable.
Among those giving evidence were former employees - who all say they warned CEO Stockton Rush of the dangers and were ignored.
