🏥 Department: STD/STI Services 🩺 Visit Type: Walk-In 🧾 Clinical Summary: Patient presented requesting sexually transmitted infection (STI) screening following a recent episode of unprotected intercourse. The patient denied any associated symptoms, including urethral discharge, dysuria, pruritus, pain, or genital lesions. Vital signs were within normal limits at the time of evaluation. Blood work was then drawn and a urine sample was taken. Laboratory testing indicated presence of chlamydial antibodies. Antibiotics were initated in clinic and the patient was prescribed a 7 day treatment course. 🔬 Tests Performed: • Urine STI Screening • Blood Work (Serologic Testing) 📊 Results: • Positive for Chlamydia (urine test) • Serology indicates presence of chlamydial antibodies 🩻 Assessment: Confirmed uncomplicated chlamydial infection. Patient currently asymptomatic with stable condition. 💊 Plan: Oral antibiotic therapy (7-day course) - first dose administered in clinic, continue remaining doses once daily as prescribed, abstain from sexual activity for 7 days and until treatment is completed, notify all recent sexual partners for testing and treatment, advised partners obtain STI and pregnancy testing where applicable, and use barrier protection (condoms) consistently moving forward. 🥼 Follow-Up: Follow up with STD/STI Services for reassessment and confirmation of infection resolution.
