Understanding the different types of human errors is crucial for identifying potential risks and improving safety in high-risk industries. The second HOP principle is that, to err is human; and error is normal, recognizes that humans are not perfect and that making mistakes is an expected part of the human condition. Human errors can be divided into two main categories of interest to high-risk industries: active errors and latent errors. Active errors are immediate errors caused by individuals, while latent errors are organizational errors that can exist within the system for prolonged periods of time. Within active and latent errors, we can further classify errors into omission errors, where necessary action is not taken, and commission errors, where the incorrect action is taken. In this context, it is important to understand different examples of high-risk industries where these types of errors can occur, to learn from them, and to implement measures that mitigate the risk of human error.
