Vague prompts are the main reason Claude Code goes off the rails. From there we build the single most important file in any Claude Code project: claude.md. You will see how to generate a strong starting point with the /init command, how to extend it with a Project Boundaries section that captures what is in and out of scope, and how to add an Environment Facts section so Claude stops assuming services and tools that are not actually part of your stack.
