As reported by Laslett et al. in 2005, the thigh thrust test has a sensitivity of 88% and a specificity of 69%, which gives it moderate clinical value. However, it is advised to perform this test as part of a cluster.
To conduct the test, have the patient lie in a supine position. Stand on their asymptomatic side, then flex the patient's leg on the symptomatic side to 90 degrees of hip flexion with slight adduction. Place your other hand over the patient's sacrum.
Next, apply longitudinal pressure through the patient's femur, which will create a shearing force at the SI joint. Apply three to six higher-velocity thrusts with gradually increasing pressure.
A positive test will result in the reproduction of the patient's familiar pain.
